Living with Heart Failure
Helping to control your condition
The Living With Heart Failure Programme is a 16-week programme in which you will receive a wide range of information to help support you in living with your condition.
Our aim is to provide support and guidance to you and your carers in order for you to gain a better understanding and control over your heart condition. This will include understanding how the heart functions and the role of the kidneys.
Our objectives
- to provide you, your family and carers with education, advice and support in order for you to become more actively involved with the management and monitoring of your condition.
- to enhance quality of life for heart failure patients by ensuring you receive an individualized patient management plan. This will enable you to play a more active role in the management of your care.
- to closely monitor patients to identify potential occurrences of a crisis and act accordingly.
- to reduce incidents of unnecessary emergency admissions to hospital and minimizing inappropriate interventions which may impact on your quality of life.
- to promote effective communication between all the members of the multidisciplinary team involved in your care pathway.
- to develop and improve palliative care services within the community where appropriate.
- to implement advanced care planning initiatives including preferred place of care and death.

If you would like to be referred to our Day Care Hospice or you are the carer or a family member of someone who you would like to refer on their behalf, then please either download our services from the website link or give us a call (01536) 747755 Mon-Fri 9-5pm. We look forward to hearing from you.
If you are looking to be referred to us for the Living with Heart Failure programme, for our Hospice at Home service or our Bereavement Support service then please speak with your GP or District Nurse as you can only be referred to us for these services via a medical professional.

- to provide a centre for the relief of pain and distress, whether physical, emotional, psychological, spiritual or social
- to create a welcoming and compassionate environment for people diagnosed with a life-threatening illness, their families and carers
- to provide holistic care to enhance quality of life, including effective symptom control and social and emotional support, enabling the person to live life as fully as possible
- to treat each individual with dignity, respect, understanding and acceptance
- to establish a resource centre for education and training, for professionals, families and carers and other support agencies involved in providing palliative care
- to provide development training and support for staff and volunteers to ensure a skilled and expert team that is enthusiastic about its role
- to work in partnership with Primary Care Trusts, General Practioners and other providers in the development of comprehensive palliative care services
- to work with Northamptonshire Provider Services in accordance with the information sharing agreement signed by both parties
- to monitor and evaluate our services and implement development programmes where appropriate
- to accept referrals from District Nurses when a patient is felt to be within the last four weeks of life
- to provide skilled nursing care to patients in their home
- to provide consecutive care every night including weekends and bank holidays (where possible) until the patient dies or care is no longer required
- to provide support and bereavement follow-up to families, carers and friends
- to secure the financial resources to run and develop the service we provide
- to ensure cost-effective use of resources

Creative Writing For Wellbeing
Writing as a form of expression can be very therapeutic and beneficial to mental wellbeing, explore different ways of writing each session. Support can be given to those that want to express themselves on paper but may need some help with the writing or literacy. Ideal for those new to Learn2b.

Corby Branch meets on the first Satuday of every month at 7.00pm at Lloyd's Road Social and Ex-Servicemen's Club.

We have volunteers in Northamptonshire who offer help and support so older people can stay safe warm and well at home across Kettering, Corby, Wellingborough, Rushden, Daventry and Northampton.

As everyone is different, Royal Voluntary Service can tailor support to your individual needs. We’ll visit you at home and work with you to create a support package that suits you.

Stages
STAGES (Support Training Advice Guidance Educational Services Limited) was initially created in August 2013 to fill the gap in after-care for recovering addicts and to help Mental Health patients to find peace and relaxation in their lives. It was also set up to help families of addicts and carers of mental health patients to recover their own lives and help themselves and their loved one on a journey to rescue their lives and give them hope of a brighter more positive future.
Why the community needs us
We offer Mindfulness Meditation Courses, Group Support, holistic therapies in Reiki, Indian Head Massage, Reflexology, Crystal Healing, Art therapy, Facebook and email support and help our clients to access any other local services that we are aware of. It is our aim to help and support people and empower them to help themselves and their families. We also have a huge collection of books relating to addiction, co-dependence and mental health which we share with our service users.
Our impact on the community
Some of our services users have reported gaining new perspectives, alternative coping mechanisms, growing in self-awareness, understanding and the ability to forgive. Gaining inner peace gentle healing and contentment. The alternative therapy treatments on offer have given much needed relaxation and relief from stress related pain.
